Document Upload
The Document Upload section includes the ability to upload required and additional documents.
Document Upload Instructions
Select the Document Type in the dropdown menu.
Click Select to search for a document.
Enter the Document Name.
Enter an Expiration Date.
Click Upload File.
Document List
The Document List section includes detailed information about documents that have been uploaded.
Document List Instructions
Click the Link in the Document Name column to view a document.
Click the Link in the Electronic Signature column to sign a document.
Click the X in the Remove column to delete a document.
Required and signature documents must be completed before moving to Review and Submit.
Document links and e-signature open in a new window. Review the document, and if required, click the Submission Agreement box(s) and the Submit button at the end of the form. Documents are removed from the required list as they are signed or uploaded.
